how to ban people

how do I ban someone from a subsaidit I moderate. Whenever I try I leave the "username" bar empty and I dont know if im supposed to or not. If not what should I write in that bar? My name? The username of the person getting banned?

The username of the person getting banned.

Also, they're just called subs, not subsaidits ;)
