This was no ordinary medical conference. Over the course of three days, I learned a great many things. That eunuchs are one of the world’s oldest gender identities and that doctors should not judge their strange desires for castration but fulfil them. That, “ideally, patients wouldn’t be actively psychotic” when they initiated testosterone, but that psychotic patients consent to take medication like stool softeners and statins all the time and “people don’t pay that much attention”. That it would be “ableist” to question an autistic girl’s insistence on a double mastectomy. That patients who claim to have multiple personalities that disagree about which irreversible steps to take toward transition can find consensus — or at least obtain a quorum — using a smartphone app.

I’m not aware of any other medical procedure that requires the approval of a therapist.

Because there aren't any other examples of permanent, life-altering surgeries doctors perform to treat a mental illness.

The author seems to be of the opinion that this was not the case for most people at the conference, although I don't know how she reached that conclusion exactly, they certainly weren't going to say "And you know, the best part about all this gender stuff is the huge amount of money we make". Still, the possibility that they're all true believers is kind of frightening.

I think she's just not in the medical field so she doesn't understand there's a process by which young idealistic doctors who want to make a difference are gradually ground down by the reality of their practice into doing whatever it takes to pay their loans off and start making house payments.

It's just not possible the endgame of this is the rich, privileged classes behind the curtain deciding to make a certain subculture group trendy and popular and raise anyone in it on a pedestal- so that they can trick the feeble-minded, the stupid or mentally disabled, and people with undesirable races or political views into castrating themselves. A bad gender clinician is easily “intimidated” by complicated patients, while a good gender clinician knows how to secure consent even in the trickiest cases. Mental health difficulties become “mental health differences”. Severe autism or thinking you have multiple personalities living inside your head become empowering forms of “neurodiversity”. When it comes to assessment, “careful” and “comprehensive” have become dirty words: “The answer always seems to be more assessment and more time. That’s gatekeeping.”

This is the ultimate goal of Queer Theory, the elimination of norms meaning that a paranoid schizophrenic is no longer seriously mentally ill and in need of psychiatric treatment, they just think in different ways. Autists don’t actually have a complex neurological disorder, they’re neurodivergent and their difficult behaviour needs to be accommodated and treated as acceptable because people with autism are just quirky like Sheldon from the Big Bang Theory.
